Job Title: Health Coach

Pay Transparency: $32-$36 hour

Role and Responsibilities

The Health Coach is responsible for carrying out Active Wellness's commitment to serve members with full commitment and attention as healers, and to deliver on Active Wellness's commitment to build healthy communities through our core values of authenticity, teamwork, innovation, versatility, and excellence. The Health Coach will work with a team, including medical providers, to plan the best course of action to help patients achieve their health goals. The Health Coach will be one of the first points of contact when patients are referred from physicians into the Hoag Lifestyle Program and will continue to be the primary point of contact for members throughout their experience in the Hoag Lifestyle Program. The Health Coach will work individually with members to support them in developing a plan to improve their health based on the prescription for exercise, nutrition, wellness education, and technology that the physician has prescribed. As part of this process, the Health Coach will help onboard members to exercise equipment available in the program, and technologies like Welld that are available to assist the member in meeting health goals by tracking their progress and results. The Health Coach will provide on-going support, guidance, and outreach to patients in the program and continue to communicate progress back to the Hoag health care team through established communication platforms and team meetings.

Specific Responsibilities

Essential Functions

  • The Health Coach services as the primary point of contact for sharing member information and progress with physicians and other medical staff.
  • Facilitate sessions with clients in person, via phone, and through email
  • Help patients, members and prospects onboard the program
  • Identify behavior and habit changes to help patients transform their lives
  • Provide motivation, guidance, support, and encouragement
  • Work one-on-one with clients to establish a personalized wellness plan to improve health metrics
  • Identify short-term and long-term exercise, nutrition, and weight management goals
  • Assist clients in making behavior changes and providing on-going follow up
  • Track improvements in health behaviors and biometric measurements using provided technology platforms
  • Facilitate educational workshops, presentations, screenings, events, health campaigns, etc.
  • Comfortable presenting in front of groups of people
  • Able to motivate individuals toward making health improvements
  • Evaluate new or existing programs and make specific recommendations for the most effective programs based on the specific needs, interests, and demographics of the member
  • Maintain communication and relationships with leaders and other health care providers in the community
  • Adhere to all Active Wellness and client policies and procedure
  • Attends applicable staff meetings and training
  • Administer emergency care to members and guests in need of assistance
  • Work as a TEAM with all other departments
  • Other duties and responsibilities as assigned by supervisor

Other Functions

  • Remain current on changes in policies, procedures and product offerings.
  • Demonstrated ability to provide expert customer service across all contact channels (e.g., chat, phone, email).
  • Display superior customer service and communication skills and always maintain professional interactions with customers and associates.
  • Be knowledgeable about all programs and activities offered throughout the center
  • Attend Active Wellness site meetings and trainings
  • Handle injury and illness and security incident reporting

Requirements

Qualifications and Education Requirements

  • Preferred degree in Health Promotion, Nutrition, Exercise Science or similar field education preferred
  • Wellness/Health Promotion Certifications, ACE Health Coach, Wellcoaches, or other health coach certification
  • 2-3 years of experience delivering nutrition, fitness, and/or coaching programs to both individuals and groups in health and fitness settings.
  • Must be detail oriented, organized and highly responsive with a commitment to customer service
  • Strong verbal communication skills
  • Basic computer skills, customer service skills, results oriented, ability to multitask.
  • Experience working collaboratively with health care to support individual patient goals and outcome
  • Working knowledge of health, nutrition, fitness, personal development, and how to facilitate habit change.
  • Knowledge of health-related diseases and illnesses, and the typical medical treatments used to address them.
  • Experience with utilizing technology to support program tracking and participant engagement.
  • Passion for helping others improve their health and wellness.
  • AED/CPR/First Aid Certified
  • National Personal Training Certification(s) required (see approved Certification, Licenses and Permits list) as well as modality specific certifications as required by role (if applicable)
  • Proof of citizenship or legal status

Physical and Working Conditions

  • Ability to take the responsibility for the health and safety of others
  • Ability to stand for several hours in a same shift; ability to lift 25 lbs
  • Fitness club environment
  • Must follow OSHA and Active Wellness safety standards
