Employment Planning & Assessment:

  • Conduct person-centered assessments to identify skills, interests, and support needs..

  • Collaborate with support coordinators, families, and interdisciplinary teams to ensure appropriate job matches.

Job Development & Placement:

  • Establish and maintain employer partnerships to create and sustain inclusive job opportunities.

  • Assist with applications, resumes, and interview preparation.

  • Support participants in understanding workplace culture, expectations, and appropriate social interactions.

Job Coaching & Retention Support:

  • Deliver on-site and off-site job coaching, fading support as participants gain independence.

  • Monitor job performance and satisfaction for both the participant and employer.

  • Advocate for reasonable accommodations and inclusive workplace practices.

  • Provide follow-up and long-term retention support in accordance with DDD service guidelines.

Compliance, Documentation, & Reporting:

  • Maintain accurate, timely, and complete documentation of services, progress, and outcomes in compliance with DDD, Medicaid, and agency standards.

  • Complete daily service logs, progress notes, and billing documentation.

  • Participate in team meetings, ISP reviews, and required trainings.

Required Certifications & Qualifications:

Education & Experience:

  • Minimum: High School Diploma or GED (Associate or Bachelor’s Degree in Human Services, Psychology, Rehabilitation, or related field preferred).

  • At least one (1) year of experience providing vocational or habilitation services to individuals with developmental disabilities.

Required Certifications:

  • Division of Developmental Disabilities (DDD) Mandatory Training Modules – completed before or within 90 days of hire.

  • Certified Employment Support Professional- Required

  • Must complete all of the training at the Boggs Center - Must have a job coach experience 

  • CPR and First Aid Certification – current and maintained throughout employment.

  • Valid Driver’s License with acceptable driving record and proof of insurance.- Required

Skills & Competencies:

  • Strong interpersonal, communication, and advocacy skills.

  • Ability to motivate and support individuals toward independence and employment success.

  • Excellent organizational skills and attention to detail for documentation and reporting.

  • Knowledge of community resources, job development techniques, and ADA employment provisions.

  • Flexibility to work varied hours, including evenings or weekends, to meet participant and employer needs.

Work Environment:

  • Primarily community-based with regular travel to employer sites, participant homes, and agency offices.

  • Must be able to lift up to 25 lbs. and assist individuals as needed in a work setting.
